One hospital in Harris County is asking its patients to secure personal information with a phone call to the IRS and the Social Security Administration after learning that personal information may have been compromised by a former employee.

According to a hospital spokesperson, several patients at the IntraCare North Hospital reported false income tax returns that were filed using their personal information. The former hospital employee that may have caused the breach was an intake coordinator for the hospital from March 15 to Aug. 18, 2011. That ex-employee had access to patients’ personal information before she was fired from the hospital. Hospital personnel did not learn of the breach until April 18th.

Although exact numbers are not known, it is believed that the former employee had access to the information and records of approximately 741 patients; the former worker had the information in a binder. Hospital officials are now working with the Harris County District Attorney’s Office.

The IntraCare North Hospital is asking those that were patients at the hospital to to secure personal information by calling both the Internal Revenue Service (1-800-908-4490) and the Social Security Administration (800-269-0721). Patients are also being urged to contact all three credit reporting bureaus (Experian, TransUnion and Equifax) to report discrepancies and issues.
